Pillar 1
Health Assessment
A thorough nurse-led health review at home within 72 hours of enrolment — so care starts with clarity, not guesswork.
Risk identification comes first
The GRIC ensures no older adult enters the programme without a structured, evidence-based evaluation. Risk stratification happens first; services are then matched to the individual.
Trained nurses and paramedics administer the checklist using plain-language questions with clear referral triggers — preventing both under-treatment of high-risk patients and unnecessary use of resources for low-risk clients.
- 15 clinical and social domains (A–L)
